Common misconception
We don’t actually know if the 4th envelope was intended for the Brigadier. The Doctor didn’t even decide to get on with sending the envelopes until after he learned the Brig had passed away. He was intent on putting the job off indefinitely to dick around, going out partying with Alistair included in “dicking around”.
